
Heart failure (HF) is a prevalent and chronic condition in the United States, often leading to high readmission rates due to factors like unclear discharge instructions (DeSai et al., 2021). This condition is particularly impactful for the older population most affected by HF. However, many HF patients, including those on 3 East, receive limited educational materials upon discharge, which may need to improve their understanding of self-care practices (Schofield et al., 2023). This highlights the urgent need for a change in our patient education approach.
